Register to be a member of St. Mary Magdalene Church

If you are new to the area, an adult child of a current/former SMM parishioner (anyone over 18 and earning an income), or have been attending Mass at SMM, you are encouraged to become a registered member of our parish.  For your convenience, you can complete the registration form online by clicking the button below. Or download HERE, print and return a paper registration to the Parish Office or drop it in the Offertory Collection basket.

Family Change Form

This form is to be used for a currently registered family with a new baby, change of address, older child no longer in the household, marriage, change of contact information, etc.

You can always call the Parish Office at (614)274-1121 to report your changes as well.
